Skilled nursing, without the Home Health box.

Godwins Family Care is hiring an LPN for skilled nursing services in client homes and facilities. You'll work within our Private Home Care Provider scope, which allows for continuous shift coverage alongside discrete skilled visits.

This is not a traditional Home Health role. Our scope permits extended-shift private duty nursing that intermittent home health agencies cannot offer.

You'll carry out provider-ordered care plans with FNP oversight and clear escalation paths.

  1. i.
    Skilled nursing visits: medication management, wound care, vital sign monitoring, treatments.
  2. ii.
    Continuous skilled care shifts for medically complex clients at home or in facility settings.
  3. iii.
    Carry out provider-ordered care plans under FNP oversight.
  4. iv.
    Document accurately and communicate clinical changes promptly.
  5. v.
    Support chronic disease management, post-hospital transitions, and end-of-life care.
  6. vi.
    Build trusted relationships with clients, families, and partner care teams.

$22 base. Premiums stack.

Base Rate
$22/ hr
Weekend shift+$2 / hr
Overnight shift+$3 / hr
Short-notice fill+$3 / hr
Complex care client+$2 / hr

W-2 employment with paid certifications and PTO accrual. FNP oversight with clear escalation protocols on every case.

Mentor & Coordinate

Mentor new nurses, support care plan reviews, and serve as point of contact for complex clients.

Stage 3

RN Bridge

Education Support

Continue working while pursuing RN. Your clinical hours and FNP supervision strengthen your application.

Stage 4

RN or Clinical Coordinator

Active Service Line

Transition directly into our skilled nursing or HBPC line. Top performers get first consideration as we grow.

What we're looking for.

Required

The basics
  • Active, unrestricted Georgia LPN license
  • Current BLS certification
  • Minimum 1 year LPN experience (home health, SNF, or hospital)
  • Reliable transportation
  • Clean background check including Georgia GChecx fingerprint clearance
  • Strong clinical judgment and ability to work independently in client homes

Preferred

Bonus, not required
  • Home health or private duty nursing experience
  • Geriatric, dementia, or behavioral health background
  • Experience documenting for LTC insurance claims
  • Bilingual capability
