Godwins Family Care LLC is a physician and family nurse practitioner-owned home care agency based in Atlanta, Georgia. We serve families across North Atlanta, Cobb County, and surrounding areas. This policy explains how we handle the information you share with us through our website, intake forms, email, and during the course of providing care.

Information We Collect

The information we collect depends on how you interact with us. At the assessment stage, we collect what you choose to share to help us understand your loved one's care needs. This may include:

  • Your name, contact information, and relationship to the client
  • Your loved one's name, address, contact information, and basic demographics
  • General information about their care needs, daily living abilities, and schedule preferences
  • Caregiver preferences and matching considerations

If you proceed to become a client, we collect additional information necessary for safe and effective care:

  • Detailed medical history, diagnoses, medications, and allergies
  • Behavioral and cognitive status, including mental health history when relevant
  • Emergency contacts and authorized care decision-makers
  • Medical provider, hospital, and pharmacy information
  • Payment information including long-term care insurance details when applicable
  • Signed consent and authorization forms

How We Use Your Information

We use the information you share for the specific purpose of evaluating, planning, coordinating, and delivering home care services. Practically, this means:

  • Determining whether our services are a good fit for your family
  • Matching your loved one with the right caregiver and care plan
  • Coordinating with your loved one's medical team where authorized
  • Communicating with you about scheduling, care updates, and billing
  • Maintaining required clinical and business records

We do not sell your information. We do not use it for advertising or marketing to outside parties.

How We Protect Your Information

We treat the information you share as confidential and use industry-standard practices to protect it. Information collected through our intake forms is transmitted over encrypted connections and stored within access-controlled systems. Only Godwins Family Care staff with a legitimate need to view information related to a specific client or prospect have access to it.

Once you become a client and Protected Health Information (PHI) is involved, your information is handled in accordance with applicable HIPAA requirements and our internal safeguards. A formal HIPAA Notice of Privacy Practices is provided at the start of care and signed before services begin.

How We Share Information

We share information only when necessary and only with parties you have authorized or as required by law. This may include:

  • Caregivers assigned to your loved one's care
  • Family members and emergency contacts you have designated
  • Medical providers, hospitals, and pharmacies you have authorized us to coordinate with
  • Long-term care insurance carriers when processing claims on your behalf
  • Service providers who help us run our business (such as scheduling, payroll, or document storage), bound by confidentiality agreements
  • Government authorities when legally required, such as in response to a court order or to report suspected abuse or imminent harm

Data Retention

We retain assessment and inquiry information for up to 24 months unless you become a client or ask us to delete it sooner. Once you become a client, we retain your records as required by Georgia state law and federal regulation, generally for a minimum of seven years following the end of services. Some records may be retained longer if required by law or for legitimate business reasons.

Your Rights

You have the right to:

  • Access the information we hold about you or your loved one
  • Correct information that is inaccurate or out of date
  • Request deletion of your information, subject to our legal record-keeping obligations
  • Withdraw consent for non-essential uses of your information at any time
  • Receive a copy of your records in a portable format

What You Choose to Share

You decide what to share with us. The intake form indicates what is required and what is optional. You may share as much or as little as you are comfortable with at the assessment stage. The more we understand, the better we can match your loved one with the right caregiver and care plan, but no answer is required to begin a conversation with us.

Updates to This Policy

We may update this policy occasionally. When we do, we will update the effective date above. If changes are material, we will notify active clients directly. We encourage you to review this policy periodically.
