Medicare home health does one specific job under strict rules — short medical visits after a qualifying event, typically ending in about sixty days. It isn’t custodial care, and it isn’t a primary care relationship. Two of the three kinds of care families actually need are Godwins.

Therapists teach families to provide care between visits. Our trained caregivers do the daily work, under FNP clinical oversight.
“Her Advantage plan kept denying it.”
Medicare Advantage often denies home health or requires a fight for approval. Our two service lines aren’t gated by prior auth.

Medicare episodes close on Medicare’s schedule, not the family’s. We don’t have a benefit period — we keep going.

What it does
Short skilled visits — nursing, PT, OT, or speech — largely focused on teaching family to take over the care
When it ends
When the episode closes — typically around sixty days, sometimes sooner
What you qualify for
A qualifying diagnosis, homebound status, and — for Advantage plans — prior authorization
How often you’re seen
1–3 short visits per week, set by Medicare’s rules
How it’s paid for
Medicare Part A or B — Advantage plans may deny or delay
Who leads the care
Your nurse practitioner, by name
What it does
Provider visits, prescribing, chronic disease management, behavioral health medication, post-hospital follow-up
When it ends
Ongoing — like your primary care relationship
What you qualify for
Nothing. If you need a provider who comes to you, you’re eligible
How often you’re seen
Scheduled visits plus as needed — with 24/7 concierge access
How it’s paid for
Medicare Part B for provider services. Commercial insurance once credentialing completes. Private pay available.

Your care team — same caregiver, every visit, under FNP oversight
What it does
Hands-on personal care, dementia & behavioral support, skilled nursing visits, and continuous shift nursing
When it ends
When you no longer need it — no benefit period
What you qualify for
Nothing. No prior auth. If you need help, we can help
How often you’re seen
A few hours a week to continuous 24/7 shifts
How it’s paid for
Private pay, long-term care insurance, VA contracts, or Medicaid waivers (pending)
